NEW EMPLOYEE PAPERWORK REQUIREMENTS

    You should have already received a packet containing the information and forms listed below.  If you have not received this packet, please contact the Office of Personnel at (864) 260-5000, ext 10157.
     
     
    In an effort to streamline the paperwork requirements for Personnel, Payroll, & Benefits; "need to know" information is listed below pertaining to each form in your packet as well as links (where applicable) to those forms in the event you should need another blank copy.
     
     
    PERSONNEL
     
    SC DHEC School Employee/Individual Certificate of Evaluation for Tuberculosis
    (This form can only be obtained from the Office of Personnel Services)
     ***The test and form must be completed by your doctor or clinic PRIOR to your first day of work.  YOU WILL NOT BE ALLOWED TO BEGIN WORK UNTIL YOU HAVE RETURNED THIS FORM TO OUR OFFICE, SIGNED AND DATED BY A PHYSICIAN, INDICATING NEGATIVE RESULTS.
     
     
    The US Citizenship & Immigration Services (USCIS) requires that all ALL employees (full-time, part-time, substitute, etc.) verify their eligibility to be employed in the United States.  The link provided here is a 9 page document.  You may read all information provided but only need to print and complete page 7. (Page 8 should also be printed, preferably on the backside of page 7, but will need to be completed by an authorized representative of the Office of Personnel Services).
    Please read carefully and follow all instructions in SECTION 1 (of page 7) ONLY.  Type or print neatly, in black ink, ALL REQUIRED FIELDS. REMEMBER TO SIGN & DATE WHERE INDICATED
    See page 9 for the document(s) you will have to present (original form-not copies) as proof of your employment eligibility. 
    NOTE:  You may choose any of the documents listed by either of the two methods listed on page 9.  However, you will be required to present your signed social security card for Payroll purposes.  In this case, it may be easier to use your driver's license (column B of page 9) and your social security card (column C of page 9) for this purpose.
     
     
    Personnel Record
    (This form can only be obtained from the Office of Personnel Services.)
    Please complete the TOP PORTION ONLY (i.e., Name in Full, In Emergency Contact, Telephone No., Address of Contact)
     
     

    PAYROLL
     
     
    (It is not necessary to complete the "Personal Allowance Worksheet" in the middle of page 1 nor the "Deductions and Adjustments Worksheet" on page 2)
    Please complete ONLY the "Employee's Withholding Allowance Certificate" at the bottom of page 1.  If you are unsure of what information to include, please consult your trusted tax professional.

    ALL EMPLOYEES are required to receive their payroll checks by direct deposit into a checking or savings account of their choice.  You also have the option of splitting your direct deposit between two accounts.
    Please read carefully and follow ALL instructions.  Type or print neatly, in black ink, ALL REQUIRED FIELDS.
    REMEMBER TO SIGN AND DATE WHERE INDICATED
    TIP: The "Depository Name" is the name of your banking institution.  The "Bank Transit/ABA Number" is the first set of numbers at the bottom left hand corner of your check (usually immediately preceding your account number).  If you are unsure, please contact your bank to obtain this information.
    You will be required to tape/staple a VOIDED check or Savings Deposit slip to this form.  If you do not have these items to attach, please obtain an "Account Verification for Direct Deposit" form from your banking institution and staple it to the Anderson 5 Direct Deposit Form.
