Immunization Requirments

Each year, the South Carolina Department of Health and Environmental Control (DHEC) provides the South Carolina Immunization Requirements for Childcare and School. Please see the links below which provide the childcare and school entry requirements for immunizations for the 2023-2024 school year.

Also, the new Immunization Certificate is DHEC form 4024; no other immunization certificate will be accepted on or after July 1, 2022.
